Description

Commonworks is an innovative, managed care approach to providing a continuum of services for adolescents aged 12-18 who are in the care or custody of Department of Social Services. It provides an integrated, comprehensive array of services for adolescents and their families through 6 networks of private providers, each under the management of a Lead Agency. The program operates on the basis that a child’s emotional and educational needs are optimally met through a system promoting stability, minimizing transitions and offering the least restrictive home and school setting.

Commonworks ensures that the clients will receive the most appropriate services in the most appropriate setting by offering an array of residential and community-based services through various levels of care, from enhanced residential care down to specialized foster care.

Population

Adolescents ages 12-18 who are in the care or custody of Department of Social Service as a result of removal from family due to abuse, neglect, disrupted adoption, voluntary placement or a Child In Need of Services (CHINS) Petition. These adolescents present a range of psychosocial, emotional, and/or cognitive problems which impair their ability to function. The nature of their needs have led to problematic behavior patterns that could not be addressed within the family.
