Description
Family Cares Network is a system of non-residential, community-based care provided to families in Central Massachusetts that are referred by the Department of Social Services. This network includes a range of agencies that contract with Department of Social Services to provide wrap around services including family evaluation and counseling, family support and stabilization services as well as after school and tracking services for children and youth.
The program model encourages family participation in a joint planning process with DSS staff, Y.O.U., Inc. service coordinators and network provider staff. This planning process assesses the family needs and strengths and develops a plan for services that the family can participate in. This model attempts to be a strength based model enabling the family to participate in a positive way. Community supports and resources are also sought out by the service coordinators and referrals are made as appropriate.
