• Service Area: Managed Care Services
• Qualifications: MA and treatment planning experience required.
• Provides clinical care coordination and support services for clients referred for services by DSS. Coordinates intake and preparation of all new referrals to Network providers. Responsible for entering and managing referrals on DSS Family Net information management system.
